Job summary

Crusader Community Health seeks a Behavioral Health Case Manager to deliver comprehensive case management and counseling across clinic and community settings. You will coordinate care for children, adolescents, and adults with mental health, substance use, and psychosocial needs, working with providers, families, and community partners.

You will advocate, educate, and link patients to services, promoting health, wellness, and transitional care while supporting patients and families with skills

Qualifications

• Bachelor’s degree in human services required
• Master’s degree in Social Work, Counseling, Psychology, or related field preferred
• CADC certification preferred
• Experience with substance use disorders required
• Experience with mental health issues required
• Case management experience preferred
• Ambulatory care experience preferred
• EHR experience preferred

Responsibilities

• Provide comprehensive case management across clinic and community settings
• Deliver behavioral health counseling and education to patients and families
• Provide patient and family skills training and support
• Promote health and coordinate transitional care
• Advocate and refer to community services and agencies as needed
