Job summary

Apis Services, Inc. is seeking a Case Manager to provide community-based support and coordination of services for program participants.

The role focuses on assessing needs, connecting individuals with resources, and supporting the implementation of individualized plans to promote recovery, stability, and independence. The Case Manager will work with multidisciplinary teams to deliver comprehensive services across housing, healthcare, employment, education, and behavioral health supports, while

Qualifications

• Bachelor’s Degree in Human Services or related field preferred
• Two (2) years of experience supporting individuals in behavioral health, addiction recovery, or intellectual/developmental disability services required
• One (1) year of case management experience preferred

Responsibilities

• Coordinate and implement individualized service and recovery plans.
• Provide community-based case management services and support participants in achieving personal goals.
• Assist participants with accessing resources such as housing, medical care, employment, education, and behavioral health services.
• Advocate on behalf of participants to ensure access to necessary supports and services.
• Transport participants to appointments and community resources when required.
• Collaborate with clinical staff, community providers, and other stakeholders.
• Maintain accurate and timely documentation in accordance with program and regulatory requirements.
• Identify service gaps and communicate recommendations for improving participant services.
• Attend team meetings, supervision, and required trainings.
