Lutheran Social Services of Illinois is seeking a dedicated professional to provide case management and help maintain a supportive, therapeutic environment. The role includes outreach to partner agencies to foster referrals and manage admissions efficiently.

Responsibilities also cover intake procedures, scheduling, and client discussions about program fees. Strong communication, organization, and computer skills are essential, along with a valid driver’s license and access to reliable

Qualifications

• High school diploma or equivalent preferred.
• Background check clearance required.
• Annual TB test required.
• Ability to effectively utilize Electronic Health Records (EHR).
• Ability to utilize NOWPOW for case management.
• Customer service experience preferred.
• Clear written and verbal communication.
• Strong organization and computer skills including MS Office.
• Valid driver’s license and reliable transportation preferred.
• Valid IL liability insurance preferred.

Responsibilities

• Provide case management for clients.
• Encourage, monitor, and maintain a therapeutic environment.
• Outreach to other agencies to increase referrals and engagement.
• Assess client’s ability to meet admission criteria and placement.
• Complete intake packets for admission into program.
• Schedule intakes and walk-throughs.
• Conduct weekly discussions with clients regarding program fees.
• Demonstrate professional, respectful behavior with clients, families, staff and partners.
