Job summary

Valley Healthcare System, Inc. seeks a Supervisor of Community Case Management to oversee programs under HUD's COC guidelines in the Chattahoochee Valley. You will lead partnerships, ensure quality case management, and guide staff toward efficient workflows and compliance.

Reporting to the CEO, this role focuses on program development, policy implementation, audits, and serving as a resource for best practices while advocating for clients and families served by VHcS.

Qualifications

• Excellent communication, listening, mediation, and writing skills.
• Strong organizational skills with ability to meet a demanding workload.
• Detail oriented to complete client files and grant reporting requirements.
• Ability to read and interpret contracts.
• Valid driver’s license.
• Experience with computers and Microsoft Office, Outlook and Teams.
• Understanding VHcS mission and goals.

Responsibilities

• Direct the development and implementation of departmental policies, procedures, and programs.
• Oversee daily operations of case management with participating agencies to ensure quality service.
• Establish protocols and benchmarks for case management success.
• Monitor staff performance and provide coaching to promote growth.
• Foster relationships with agencies in Muscogee and Russell Counties.
• Advocate for clients to ensure needs are met.
• Serve as a resource on best practices and ethics for case managers.
• Conduct audits to ensure documentation meets quality standards.
