Job summary

Valley Healthcare System, Inc. in Columbus, GA is seeking a Housing Case Manager to assist clients moving from homelessness into permanent housing and to prevent homelessness.

You will provide housing search support, develop and monitor Individual/Household Service Plans, and coordinate with partner agencies while maintaining audit-ready client files. This on-site role requires strong advocacy, documentation, and collaboration with landlords.

Qualifications

• Certified Peer Specialist certification required.
• BSW or higher in social/human services preferred.
• Two years of case management experience preferred.
• Experience with homeless individuals and families beneficial.
• Knowledge of community resources and referrals required.

Responsibilities

• Identify households at greatest risk and determine needed support to mitigate homelessness.
• Establish relationships with agencies and inform them of mainstream services.
• Recruit and maintain relationships with private landlords to participate in renting to clients.
• Assist clients to develop and monitor progress in meeting goals of Individual/Household Service Plan.
• Request payments on behalf of clients (security deposits, utility deposits, rent).
• Provide advocacy and negotiation with landlords/rental managers to enable program participation.
• Collaborate with partners to implement client plans for obtaining and maintaining housing.
• Inspect dwellings to ensure Housing Quality Standards.
