Job summary

PVH Potomac Valley Hospital in Keyser, WV seeks a Supervisor of Community Health to oversee Community Health Worker staff, scheduling, payroll, community programs and preventive medicine assignments, and data collection.

The role collaborates with Preventive Medicine leadership to improve performance and patient safety, and to help clients access a medical home and chronic care management resources across diverse communities.

Qualifications

• High School diploma or equivalent required.
• Valid Driver’s License.
• CPR Certification required.
• Three years of experience as a Community Healthcare Worker or in a healthcare setting.
• Preferred: some vocation/college courses; experience with Electronic Health Records.

Responsibilities

• Supervises Community Health Worker staff including scheduling, payroll, community programs and data collection.
• Assists with monitoring performance improvement and hospital-wide requirements for employees, patient care, and safety.
• Assists clients in identifying health-related services and establishing a medical home.
• Coaches patients in managing chronic conditions and self-care.
• Helps clients design and maintain personal health records and health plans.
• Educates community members on health screenings and immunizations; promotes routine check-ups.
