Qualifications

• Bachelor’s degree in health sciences, social work, counseling, or related field.
• Master’s degree is a plus for advanced leadership.
• LCSW/LMSW and CSWCM certifications are advantageous.
• CPR certification is a plus for clinical oversight.

Responsibilities

• Oversee daily EDWP operations, resource allocation, staffing, and program performance.
• Lead and support Team Leads and Case Managers through coaching and professional development.
• Monitor KPIs, identify variances, and implement quality improvement actions.
• Provide clinical oversight via complex case consultations and joint home visits.
• Coordinate care with physicians, nursing leadership, and community stakeholders.
• Support recruitment, onboarding, and ongoing staff development.
• Promote culturally responsive, member-centered care and integration of services.
• Ensure compliance with EDWP, Medicaid/Medicare, and VNHS policies.
