Job summary

Volunteers of America Delaware Valley is seeking a dedicated outreach/CARE coordinator to work with the IMPACT team in designated communities. You will provide information, assess emergency needs, and deliver direct services while promoting client self-determination and integration into treatment and community self-help activities.

Qualifications

• High school diploma or equivalent and at least two years in a relevant field.
• Experience with homeless populations and individuals facing substance abuse or mental health challenges is preferred.
• Effective written and verbal communication skills are essential.

Responsibilities

• Participate in regular street outreach to engage homeless individuals with program and law enforcement outreach.
• Disseminate information about agency services to the community.
• Coordinate transportation for individuals requesting services.
• Conduct comprehensive intake assessments for residents from outreach activities.
• Communicate special needs and advocate for clients with program staff.
• Network with community agencies to develop client resources.
• Develop Service Plans with client input and approved tools.
• Monitor client progress and document outcomes per funding standards.
• Develop discharge plans aligned with contractual requirements and policies.
• Complete required reports and case-related paperwork promptly.
• Facilitate regular group sessions for clients as directed.
• Provide practical assistance and mentorship to clients entering the facility.
