Job summary

Volunteers of America Delaware Valley is seeking an Aftercare Treatment Specialist to support individuals transitioning from structured supervision to independent living in Camden, NJ. You will assess risks, develop individualized service plans, provide counseling, and ensure continuity of care after release.

In this role you will conduct assessments, lead case management sessions, connect clients with housing, employment, and mental health resources, attend hearings, and support ongoing program

Qualifications

• Bachelor’s degree in a Human Services field is required.
• At least 2 years of professional mental health experience.
• Knowledge of addictive behaviors and group dynamics.
• Strong written and verbal communication skills.
• Basic computer skills.

Responsibilities

• Conduct client assessments and create recovery-focused service plans.
• Facilitate one-on-one case management and group workshops.
• Monitor progress, document outcomes, and update treatment plans.
• Link clients to housing, employment, and mental health services.
• Attend court/parole hearings and comply with funder reporting.
• Coordinate discharge planning and long-term support strategies.
• Participate in team meetings and trainings.
• Perform drug and alcohol screenings and maintain confidential records.
• Support program operations and assist with data and reporting.
