Lincolnshire County Council is seeking two Community Care Officers Level 2 to join the Adult Care and Community Wellbeing Team (Lincoln North and Gainsborough). The roles cover full-time (37 hours) and part-time (18.5 hours) with hybrid working, requiring travel across the county and mileage reimbursement from the office to appointments.

You will conduct Adult Needs Assessments and Reviews under the Care Act 2014.

Qualifications

• Experience in strength-based assessment including risk assessments and care planning.

Responsabilités

• Complete Adult Needs Assessments and Reviews in line with the Care Act 2014.
• Support individuals to achieve aspirations, access communities and promote independence.
• Deliver person-centred, solution-focused support and collaborate with the Wellbeing Team.
