Lincolnshire County Council is seeking two Community Care Officers Level 2 to join the Adult Care and Community Wellbeing Team (Lincoln North and Gainsborough). The roles cover full-time (37 hours) and part-time (18.5 hours) with hybrid working, requiring travel across the county and mileage reimbursement from the office to appointments.
You will conduct Adult Needs Assessments and Reviews under the Care Act 2014.
We are delighted to be able to offer two positions for the role of Community Care Officer Level 2 (CCO) within our Adult Care and Community Wellbeing Team (Lincoln North and Gainsborough). We are looking for individuals to fill a full time position (37 hours per week) and a part time position (18.5 hours per week). The positions are available due to internal progression and development. A Community Care Officer level 2 (CCO) plays a vital role in supporting adults with identified care and support needs, ensuring the right support is received at the right time to enable independent living. This role is central to delivering statutory duties under the Care Act 2014.
As part of the Community Wellbeing Team, you will work with individuals who require guidance and support. Our goal is to support and enable people to achieve their aspirations, access their communities, and promote independence. Working in a person-centred, strengths-based approach, you will complete Adult Needs Assessments and Reviews in line with the Care Act, focusing on what individuals can do rather than what they cannot.
You must have the ability to travel across the county within reasonable timeframes in order to meet the needs of the service. This may include responding to unplanned visits at short notice, which could involve travel outside the local area or county. Mileage expenses are reimbursed for travel from the office to appointments. Our core office hours are 08:45-17:15 (Mon-Thu), 08:45-16:45 (Fri). We work in a hybrid way with a mix of home based and office working. We currently work from offices for a minimum of two days per week. However, there will be a requirement to be in the office to meet the needs of the service.
Within the Community Wellbeing Team team we are proud of the work we complete and the high-quality service we deliver. Recruiting the right person is essential to ensure we continue to provide the best possible support to those who access our services as well as our colleagues.
We are committed to Making a Difference across Lincolnshire.
You will need to have experience in strength-based assessment including risk assessments and care planning in order to support our goal to deliver person-centred, solution-focused support.
We celebrate our team and individual achievements and share our experiences. It is important that practitioners feel appreciated, motivated and supported - our staff are our most valued asset. We offer monthly 1-1 supervisions, team and development meetings, extensive training and learning, and opportunities to help shape the delivery of services.
By joining us, you will help us achieve our shared vision: empowering people, promoting independence, and improving lives across Lincolnshire.
In accordance with Part 7 of the Immigration Act 2016 (Fluency Duty), the ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post.
