Qualifications

• High School Diploma or GED.
• Certified Community Health Worker in Maryland.
• Residency within the target population preferred.
• Experience as a community health advocate.

Responsibilities

• Advocates for individual and community health equity; assists patients in accessing health-related services and care home, clinic, or community settings.
• Documents encounters, reports, and maintains electronic client files to track progress and outcomes.
• Educates clients on proper use of health system services, coaches on chronic condition management and self-care.
• Links community members with federal, state, private or nonprofit programs and coordinates care plans.
