Job summary

Connecticut Community Care is seeking a Care Manager to help individuals make informed decisions about Long-Term Services and Supports, coordinating formal and informal caregivers in a person-centered model that respects consumer choices and rights.

The role involves conducting comprehensive community-based assessments, educating clients on options, and ensuring timely documentation and service delivery within DSS guidelines, with an emphasis on advocacy and collaboration.

Qualifications

• Bachelor's degree in administration, social work, public health, recreation, psychology, counseling, gerontology, or related field required.
• Minimum of two years' experience in health care or human services; substitution rules apply for certain degrees.
• Knowledge of Medicaid and Medicare funding sources for care coordination.
• Experience conducting comprehensive, systematic assessments in community settings.
• Strong advocacy and communication skills with consumer self-determination emphasized.
• Reliable transportation, valid driver's license, and current automobile insurance.

Responsibilities

• Conducts comprehensive, systematic, assessments that are person-centered with individuals, including family or representative as requested, in the person's preferred setting for the discovery, use, and screenings for public programs.
• Educates individuals on program components, service options, DSS guidelines, eligibility, costs, and benefits of each option.
• Promptly completes all client documentation, applications, forms, and additional documentation as required.
• Monitors and reviews cost effectiveness, quality and appropriateness of care plans, service orders, renewals, and contractual obligations.
• Conducts person-centered telephone and in-person interviews with clients and families for reassessment and plan adjustments.
• Works with interdisciplinary team and external resources; participates in on-call services and acts as backup for emergencies.
• May participate in mentoring new staff and continuing education.
