Job summary

CINQCARE in Maryland is seeking a Care Experience Specialist who meets people where they live and work to reduce barriers to care.

You’ll partner with nurses and social workers, listen deeply, and translate needs into care guidance. Field visits and in-home outreach are part of the role.

This role emphasizes empathy, presence, and community trust to improve health outcomes for high-need populations. A valid driver's license and travel are expected, and bilingual candidates are a plus.

Qualifications

• Must have experience in community outreach or health education.
• Valid driver's license and reliable transportation.
• Background check and training required prior to deployment.
• Bilingual or multilingual is a plus.
• Associate degree or equivalent experience preferred; Bachelor's encouraged.

Responsibilities

• Engage Family Members where they live, worship, and gather.
• Conduct in-person and phone outreach with high-risk individuals.
• Build trust through empathetic, culturally aligned communication.
• Address barriers to care such as food insecurity and transportation.
• Assist with paperwork for social services and basic needs.
• Coordinate with care teams and refer to interdisciplinary supports.
• Document interactions in CINQIQ and track engagement.
• Attend visits to support trust-building.
