Job summary

CINQCARE is seeking a field-oriented outreach role to bridge human and medical care for high-need populations. You will engage Family Members where they live, attend to barriers, and coordinate with nurses and social workers to ensure holistic support.

The position emphasizes cultural alignment, consistent presence, and empathetic communication, with in-office and in-home duties. Valid driver’s license and a 1–3 year background in outreach are required.

Qualifications

• Associate degree or equivalent; B.S./B.A. preferred.
• 1–3 years in community outreach, health education, or related work.
• Valid driver's license and reliable transportation required.

Responsibilities

• Engage Family Members where they live, worship, and gather.
• Conduct in-person and phone outreach with high-risk or recently discharged members.
• Build trust through empathetic communication and consistent presence.
• Surface barriers to care and refer for social services as needed.
• Assist with paperwork for social services and basic support needs.
• Support in-home technology setup to enable virtual visits.
• Provide warm handoffs and referrals to care team members.
• Coordinate outreach frequency and in-home visits as needed.
• Collaborate with nurses, social workers, and others during planning.
• Refer to interdisciplinary care team and attend visits when appropriate.
• Document interactions in CINQIQ and track engagement levels.
• Represent CINQCARE at community events and map trusted contacts.
