Job summary

CINQCARE is seeking an Outreach Specialist to bridge human and medical care for families in need. You will engage individuals in their communities, conduct outreach, and coordinate resources to support care access.

You will work across care teams, assist with paperwork, and help connect patients with social services, transportation, and housing supports. This role emphasizes trust-building and culturally aware communication.

Qualifications

• Associate degree or equivalent required; B.S./B.A. preferred.
• 1–3 years in community outreach, health education or related work.
• Valid driver’s license and reliable transportation.
• Background check and training prior to field deployment.
• Bilingual or multilingual skills are a plus; EMT or Social Worker experience preferred.

Responsibilities

• Engage Family Members where they live, worship, and gather.
• Conduct in-person and phone outreach with high-risk individuals.
• Build trust through consistent presence and culturally aligned communication.
• Surface and address barriers to care such as food, housing, and transportation.
• Assist Family Members with paperwork for social services.
• Support in-home technology setup for virtual visits.
• Provide warm handoffs and referrals to the care team and follow up.
• Be a consistent point of contact and adjust outreach frequency as needed.
• Collaborate with nurses, social workers, and other team members.
• Refer to interdisciplinary care and attend visits to build trust.
• Document interactions and track trust and engagement in CINQIQ.
• Represent CINQCARE at community events and map trusted contacts.
