Centerstone of Florida is seeking a Case Manager to provide mental health and substance use services, coordinating care with clients in offices, hospitals, and homes. The role emphasizes linkage to resources and ensuring continuity of care for individuals with co-occurring disorders.

You will perform assessments, develop treatment plans with input from clients and families, and maintain timely EMR documentation. A valid FL driver’s license and local community collaboration are needed for success.

Qualifications

• Bachelor's degree required with strong organizational and communication skills.
• Experience with mental health and/or substance use disorders is preferred.
• Ability to develop supportive relationships while maintaining professional boundaries.

Responsabilités

• Provide individual, family, and group case management for mental health and substance use disorders.
• Link clients and families to needed resources and coordinate care with partners.
• Complete documentation in Electronic Medical Records in a timely manner.
• Coordinate with local authorities and stakeholders to maintain treatment continuity.
• Deliver outreach services in office, hospital, and client environments.
