Job summary

Centerstone is seeking a Case Manager to coordinate services for patients, bridging mental and physical health care with coaching and education. This role supports clients in clinics, homes, and community settings, offering resources to improve overall wellbeing.

The ideal candidate has a bachelor’s degree in psychology, social work, or a related field, plus experience with electronic health records and a valid driver’s license. A rewarding opportunity for growth in a nonprofit health system.

Qualifications

• Bachelor’s degree in psychology, social work or related field is required.
• Experience with Electronic Health Records is preferred.
• Valid driver’s license in good standing required.

Responsibilities

• Help patients navigate the health care system through coaching, education and support.
• Coordinate the behavioral and primary care needs and develop strategies to remove barriers to wellness.
• Refers, advocates, and links client and family for additional services as identified.
• Use behavioral management techniques, crisis intervention, active listening and motivational interviewing to help patients develop better life management skills.
• Provides supportive and educational case management services to develop daily living and coping skills.
• Responsible for developing, implementing, evaluating, and updating treatment plans with input from the client and family.
• Enters written documentation into Electronic Medical Record in a timely manner for all services rendered.
• Connect with clients face to face in the office and community, telehealth, and by phone to address needs.
• Demonstrate knowledge and understanding of ethical standards by evidence of practice with patients and community partners.
• Attend weekly staffing and team meetings and completes assessments.
• Assist clients with developing natural support systems.
• Completes all required agency trainings to maintain 100% compliance.
• Maintains departmental productivity expectations.
• Attend and participate in clinical aupervision.
