Job summary

Centerstone is hiring a Case Manager to coordinate services and care for patients, providing coaching and resources to improve wellness. You will work across clinic, home, and community settings to address whole-person health and support recovery.

Bachelor’s degree in psychology, social work, or related field is required, with experience in electronic health records and a valid driver’s license. We offer comprehensive benefits and a collaborative, trauma-informed team.

Qualifications

• Bachelor's degree in psychology, social work or related field required.
• Experience with Electronic Health Record preferred.
• Valid driver's license in good standing required.

Responsibilities

• Help patients navigate the health care system through coaching, education and support
• Coordinate the behavioral and primary care needs and develop strategies to remove barriers to wellness
• Refers, advocates, and links client and family for additional services as identified.
• Use behavioural management techniques, crisis intervention, active listening and motivational interviewing to help patients develop better life management skills
• Provides supportive and educational case management services to develop daily living and coping skills.
