This full-time professional role covers assessment, planning, implementation, and ongoing evaluation of services to meet health and human service needs. You will coordinate with offsite supports, maintain caseloads, develop individualized service plans, document activities, and collaborate with a treatment team to promote client wellness and autonomy

Qualifications

• Bachelor’s degree in Behavioral Science or related field required.
• 2 years of experience with youth and families preferred.
• Experience in child welfare, behavioral health, juvenile justice, or residential services preferred.

Responsibilities

• Assess, plan, implement, monitor, and evaluate client care.
• Coordinate safe, timely, equitable, client-centered care with offsite support.
• Maintain caseload and monitor client progress with regular contacts.
• Draft service plans and review progress for potential case closure.
• Provide crisis intervention, advocacy, transportation, budgeting, and housing assistance.
• Document clinical activities to meet standards and funder criteria.
• Collaborate with treatment team and lead groups if assigned.
• Participate in care coordination and information sharing.
