Responsibilities

• Coordinate and provide client care that is safe, timely, effective, efficient, equitable, and client-centered.
• Maintain caseload and monitor progress through regular client contact.
• Draft service plans and determine case closure in consultation with supervisor.
• Deliver ongoing services including crisis intervention, advocacy, and referrals to community resources.
• Support wellness and autonomy through counseling and care coordination.
• Develop individualized treatment or service plans and ensure compliance with standards.
• Document all clinical activities accurately and timely.
• Participate in treatment teams and recommend client status changes.
• Educate clients and facilitate group curricula per program requirements.
