Job summary

Qualifications

  • Bachelor's degree in a related behavioral health field.
  • Valid driver's license, reliable transportation, and proof of insurance.

Responsibilities

  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
  • Maintain caseload and regular contact with clients to monitor progress.
  • Draft service plans and review progress for case closures in consultation with supervisor.
  • Provide ongoing services including crisis intervention, advocacy, medication monitoring, transportation assistance, and housing referral.
  • Assist clients in achieving wellness and autonomy through supportive counseling and care coordination.
  • Develop and implement individualized client treatment or service plans and ensure policy compliance.
  • Document all clinical activities timely to meet program and funder criteria.
  • Participate in treatment team and provide recommendations for client status changes.
  • Provide client education and facilitate group curriculum as required.
