Job summary

BlueCross BlueShield of Tennessee seeks a dedicated Support Coordinator to empower individuals with I/DD to live independently and pursue employment. The role covers Greene, Hamblen, and Jefferson counties, with travel to members' homes and mileage reimbursement.

Scheduling is 8:00am–5:00pm; candidates in Greene/Hamblen/Jefferson counties are preferred. Responsibilities include intake, enrollment, assessments, and developing Person-Centered Plans while coordinating with clinical teams to meet

Qualifications

• RN with CDDS and/or DDC certification preferred; or a related BS in health care.
• Licensed MSW or LCSW preferred; or QDDP/QIDP per federal requirements.
• In lieu of a degree, 5+ years of independent SC or case management experience.
• Healthcare/operations experience required, 2+ years.

Responsibilities

• Partner with members and families to identify supports and services.
• Collaborate with clinical and social support colleagues to meet needs.
• Conduct face-to-face visits to assess status and needs.
• Authorize appropriate home and community-based services as needed.
• Maintain a valid driver's license and drive as required.
• Travel to member homes; mileage reimbursed.
