Job summary

BlueCross BlueShield of Tennessee, Inc is seeking a dedicated Support Coordinator for the ECF Choices program in Montgomery County, TN. You will empower members to achieve better health and employment goals through holistic support across physical, behavioral, and social needs.

Responsibilities include intake, enrollment, assessments, and developing Person-Centered Plans while coordinating with a clinical team. Travel to member homes is required; mileage is reimbursed.

Qualifications

• RN with a current license preferred; CDNN or DDC credentials for related roles encouraged.
• BS in social work, nursing, education, human services, or health care field; LCSW or LCSW preferred.
• QDDP/QIDP eligibility or 5+ years as a 1915(c) HCBS coordinator with related trainings.

Responsibilities

• Conduct intake, enrollment, and ongoing assessments for members.
• Develop and maintain Person-Centered Plans.
• Support members’ health, employment, and independent living goals.
• Travel to members’ homes; mileage reimbursed; document visits and outcomes.
