Job summary

BlueCross BlueShield of Tennessee seeks a CHOICES Care Coordinator to improve members' health and independence through in-home visits and community supports. You will manage diverse requests, coordinate with clinical and social teams, and ensure safety and well-being for members in home and community settings.

The role requires RN with TN license (or compact-state) or MSW with an unrestricted license. Strong time management, communication, and documentation skills are essential for success.

Qualifications

• RN with TN license or compact-state license.
• MSW with active unrestricted license (LCSW/LMSW/LAPSW).
• Strong customer service and communication skills.

Responsibilities

• Partner with members and families to identify supports and services to meet goals.
• Collaborate with clinical and social support colleagues to meet health and living needs.
• Conduct thorough face-to-face visits to assess status and needs.
• Authorize home and community based services when appropriate.
• Maintain valid driver’s license and meet health/safety requirements.
• Complete digital literacy assessments as part of the role.
