Job summary

BrightPath is seeking a Designated Coordinator to oversee community-based services for adults with disabilities in the Twin Cities area. This full-time role blends program coordination, staff supervision, direct service, and 245D compliance to support individualized plans and high-quality outcomes.

You will lead teams, manage service planning, and ensure regulatory alignment while collaborating with guardians, case managers, and providers to deliver person-centered supports.

Qualifications

• Bachelor’s degree plus at least one year of full-time direct care experience serving people with disabilities or adults 65+, or an Associate degree with at least two years of qualifying experience, or a Diploma with at least three years of qualifying experience, or a combination meeting 245D requirements.
• Experience with Minnesota 245D licensing requirements is preferred.
• Strong written and verbal communication skills and ability to manage multiple responsibilities.

Responsibilities

• Coordinate services for individuals receiving community-based supports.
• Conduct client intakes, admissions, assessments, and service planning activities.
• Develop, implement, review, and update individualized service plans.
• Attend support team meetings and liaise with guardians, case managers, and providers.
• Maintain required client service agreements, reports, and monthly summaries.
• Support incident response and required reporting.
• Help ensure services remain person-centered and aligned with individual goals.
• Provide supervision and support to staff; conduct performance reviews and coaching.
• Train staff on client-specific supports and shadow shifts.
• Develop relationships with counties and community partners.
• Participate in on-call rotation and emergency assistance program.
