Job summary

BrightPath is seeking a Designated Coordinator to support high-quality, person-centered community-based services for adults with disabilities. This hands-on leadership role combines program coordination, staff supervision, direct service, and 245D compliance.

The Designated Coordinator works with individuals, support teams, guardians, and providers to ensure services align with Coordinated Service and Support Plans and program outcomes.

Qualifications

• Bachelor’s degree in a human services-related field with 1+ year of direct care experience.
• Associate degree in a human services-related field with 2+ years of direct care experience.
• Diploma in a human services-related field with 3+ years of direct care experience.
• Combination of education/training and supervised experience meeting Minnesota 245D requirements.

Responsibilities

• Coordinate services for individuals receiving community-based supports.
• Conduct client intakes, admissions, assessments, and service planning.
• Develop, implement, review, and update individualized service plans.
• Attend support team meetings and liaise with guardians, case managers, and providers.
• Maintain required client service agreements, reports, and program documentation.
• Support incident reporting and ensure compliance with requirements.
• Ensure services remain person-centered and aligned with individual goals.
• Provide supervision and support to IHS Specialists and other staff.
• Manage staffing patterns and conduct regular 1:1s and coaching.
• Train staff on client supports and service requirements.

Designated Coordinator for Community-Based Services

Location: Twin Cities Metro

Salary: $60,000–$70,000 annually

Job Type: Full-Time, Exempt

BrightPath is seeking a Designated Coordinator to support the delivery of high-quality, person-centered community-based services for adults with disabilities.

This role combines program coordination, staff supervision, direct service, and 245D compliance. The Designated Coordinator works closely with individuals served, support teams, and direct support staff to ensure services are delivered according to each person’s Coordinated Service and Support Plan, while helping program teams provide consistent, individualized support.

What You'll Do

Program & Client Services

• Coordinate services for individuals receiving community-based supports
• Conduct client intakes, admissions, assessments, and service planning activities
• Develop, implement, review, and update individualized service plans
• Attend support team meetings and serve as a liaison with guardians, case managers, and other providers
• Maintain required client service agreements, reports, monthly summaries, and other program documentation
• Support incident response and required reporting
• Help ensure services remain person-centered and aligned with individual goals

Staff Leadership & Development

• Provide supervision and support to IHS Specialists and other assigned program staff
• Maintain staffing patterns based on client needs
• Conduct regular 1:1 meetings, performance reviews, coaching, and performance improvement as necessary
• Train staff on client-specific supports, person-centered practices, shadow shifts, and service requirements
• Support staff competency so employees are prepared to work independently with individuals receiving services
• Facilitate communication between program staff and the management team

245D Compliance & Quality

• Ensure services are delivered in accordance with Minnesota 245D requirements
• Monitor completion of required annual staff training
• Support compliance with service planning and intensive support service requirements
• Conduct client file, documentation, and assessment audits
• Maintain required program documentation and compliance deadlines
• Identify opportunities to improve service quality and address compliance concerns proactively

Program Operations

• Complete weekly dashboard metrics and Designated Coordinator checklists
• Participate in management meetings and organizational planning
• Support development and maintenance of program policies and procedures
• Stay informed of regulatory and legislative changes affecting services
• Develop relationships with counties, regions, and community partners to support program development
• Participate in BrightPath's on-call rotation and 24-hour emergency assistance program

Direct Service

This is a hands-on leadership position. The Designated Coordinator is expected to provide direct care based on service line needs and scheduling.

Required Qualifications

• Bachelor’s degree in a human services-related field and at least one year of full-time direct care experience serving people with disabilities or adults age 65 or older;
• Associate degree in a human services-related field and at least two years of qualifying full-time direct care experience;
• Diploma in a human services-related field from an accredited postsecondary institution and at least three years of qualifying full-time direct care experience; or
• A combination of human services education/training and qualifying supervised experience meeting Minnesota 245D requirements.

Additional Experience & Skills

• Supervisory and/or leadership experience in a social service organization
• Experience working with Minnesota 245D licensing requirements
• Strong written and verbal communication skills
• Ability to organize, prioritize, and independently manage competing responsibilities
• Sound judgment and the ability to make decisions with appropriate discretion
• Ability to adapt effectively as client, staffing, and program needs change
• Willingness to learn and apply new approaches and practices
• Valid driver’s license, reliable transportation, current auto insurance, and ability to meet BrightPath driving requirements.
