Job summary

LearnAbility seeks a Designated Coordinator to oversee services for individuals under Minnesota’s 245D HCBS program. You will lead a team, collaborate with families and guardians, and ensure person-centered, compliant care aligned with Individual Support Plans.

Responsibilities include supervising DSPs, coordinating care plans, training, and maintaining safety and documentation across the program. This is a full-time on-site role based in Minnesota.

Qualifications

• A baccalaureate degree in a human services field and 1 year of full-time direct care experience.
• An associate degree in human services with 2 years of direct care experience.
• A diploma in human services with 3 years of direct care experience.
• 50 hours of education and training related to human services and 4 years of direct care experience.
• Previous supervisory or management experience in developmental disabilities or residential care.
• Strong understanding of person-centered planning and support methodologies.
• Excellent communication, problem-solving, and conflict-resolution skills.
• Ability to work collaboratively with diverse teams and maintain a positive work environment.
• Proficiency in relevant computer applications for record-keeping, reporting and time card approval/EVV.

Responsibilities

• Oversee scheduling, EVV compliance, and 245D paperwork and documentation.
• Supervise a team of support staff and provide guidance and feedback.
• Develop individualized care plans with guardians and support teams.
• Oversee behavioral support strategies and ensure adherence to protocols.
• Manage daily schedules balancing preferences, community engagement and skill development.
• Coordinate scheduling per individual and company needs.
• Train new staff and conduct regular meetings on program updates.
• Ensure compliance with regulatory guidelines and maintain records.
