Carolina SeniorCare in New Bern, NC is seeking an RN Care Manager to join the team. The role focuses on delivering holistic, person-centered care within the IDT framework, coordinating services with primary care teams, and triaging participants as needed.

The RN Care Manager serves as the primary medical contact for the Care Management Team alongside a MSW and a Provider. The position emphasizes leadership in nursing care, medication management, and ongoing participation in utilization review

Qualificações

• Graduate of accredited school of nursing.
• Bachelor’s degree in Nursing preferred.
• Current Nursing License.
• CPR/First Aid certification (Will be provided if not certified).

Responsabilidades

• Participates in care planning and implementation, preparation for daily Interdisciplinary Team (IDT) conference, as well as monitors and coordinates participant follow-up with other care providers.
• Provides nursing oversight and direct care for the participants care in the clinic as well as at home.
• Provides triage for participants that are attending the day center.
• Actively participates in Care Management Team to develop, implement, and revise policies and procedures and on-going operations decisions related to care management.
• Works closely with the Quality Coordinator in an effort to track and trend data, develop and implement quality improvement initiatives, and audit new initiatives.
• Assists participant with medication management and adherence.
• Participates in on-call rotation.
• Performs other duties as assigned by supervisor.

Formação académica

Bachelor's degree in Nursing
