You will perform initial assessments, supervise in-home caregivers, coordinate with the IDT, and provide hands-on nursing when needed.

Qualifications

• RN licensure in the state of application.
• At least 2 years of geriatrics/home care/care management experience.
• Strong communication and interpersonal skills.
• Ability to work in a team and interface with families, physicians and attorneys.

Responsibilities

• Provide oversight to all in-home caregivers.
• Perform routine in-home supervisory visits per State Regulations.
• Mentor and guide caregivers within their assigned region.
• Provide education to caregivers, staff, clients, and others on clinical issues.
• In-Home Nursing; provide some hands-on nursing tasks.
• Wound Care.
• Medication Administration.
• Medication Box setup and refill visits.
• Catheterizations.
• PIV/PICC dressing changes.
