Job summary

Kintegra Health in Charlotte, NC seeks a nutrition professional to provide nutrition care and education to Senior TLC participants in a home/community-based care model. You will evaluate nutritional status, develop and implement care plans, and coordinate with primary care providers.

Requirements include a Bachelor's in nutrition from an ADA-accredited program, ADA registration, and at least 1 year of experience with frail elderly populations; a Master's degree is preferred.

Qualifications

• Bachelor's in nutrition from an ADA-accredited program.
• Master's degree in nutrition preferred.
• At least 1 year of experience with frail elderly populations.
• ADA registered/licensed as required.

Responsibilities

• Performs nutritional assessments and reassessments.
• Provides nutrition education to participants and caregivers.
• Works collaboratively with the care team.
• Assists with coordination of 24-hour care delivery.
• Documents participant changes in the medical record and communicates with the team.
• Updates care plans throughout reassessment periods.
• Participates as a member of the Interdisciplinary Team (IDT).
