Trilogy, Inc. is seeking a Recovery Counselor - Substance Use Specialist (ACT Sycamore) to assess clients and plan individualized treatment across home, community, and clinic settings in Chicago.

You will provide motivational support, link to community services, and advocate for clients using a harm-reduction approach. The role requires SAS credential within 12 months, strong collaboration with a multidisciplinary team, and availability for evenings/weekends.

Formación

• Bachelor’s degree or two years of supervised clinical experience in mental health.
• Minimum one year of experience with clients with substance abuse issues.
• Valid Illinois driver’s license with daily access to a vehicle and appropriate insurance.

Responsabilidades

• Assess client’s personal, medical, emotional, social and environmental situation during home, community, and office visits.
• Coordinate the development of an individualized recovery plan with the client and care providers.
• Facilitate education around substance use, dual diagnosis, and harm reduction strategies with clients and team.
• Provide one-on-one services focused on substance use goals and recovery support.
• Acquire a SAS credential within 12 months of hire.
• Provide crisis/on-call coverage as directed.
• Document all services per agency policy and state requirements; maintain shared caseload.
• Assist with linking clients to community services and scheduling important appointments.

Educación

Bachelor’s degree
