Qualifications

• Experience providing case management or behavioral health services to individuals with severe and chronic mental illness.
• Experience with PRP and/or MTS-like services preferred.
• Strong interpersonal, verbal, and written communication skills.
• Ability to work effectively with individuals with complex behavioral health and social service needs.
• Proficiency with technology and basic office productivity software.
• Experience using electronic health records (EHRs) and completing electronic documentation.

Responsibilities

• Provide direct support, case management, and rehabilitation services to program participants.
• Develop and maintain professional relationships with participants to build trust.
• Assist participants with identifying needs, setting goals, and connecting to resources.
• Accompany or transport participants to appointments and community activities as appropriate.
• Assist with accessing social services, housing resources, healthcare, transportation, and other supports.
• Provide services in community settings and participants' homes when appropriate and safe.
• Support participants in developing skills for independence and community living.
• Complete timely documentation in EHRs and other systems.
• Maintain confidentiality and comply with program requirements.
• Participate in team meetings and care coordination activities.
• Develop relationships with community organizations and referral sources.
