Community Support Worker

Program: Psychiatric Rehabilitation Program (PRP) / Mobile Treatment Services (MTS)
Reports To: Clinical Coordinator

Position Summary

We are seeking a Peer Support Specialist Case Manager to join our team at Imind, in supporting individuals who are part of the priority population—individuals living with severe and chronic mental illness and enrolled in PRP and/or MTS programs.

The ideal candidate brings experience working with individuals with significant behavioral health needs, possess strong case management and engagement skills, and a genuine commitment to helping participants achieve greater stability, independence, and community integration.

This position is highly community-based. The successful candidate must be comfortable providing services in community settings and, when appropriate and safe, in participants' homes, while maintaining professional boundaries and following all safety protocols.

Key Responsibilities
  • Provide direct support, case management, and rehabilitation services to program participants.
  • Develop and maintain positive, professional relationships with participants while building trust and rapport.
  • Assist participants with identifying needs, setting goals, and connecting to appropriate resources and services.
  • Accompany or transport participants to appointments and other community-based activities, as appropriate.
  • Assist participants with accessing social services, benefits, housing resources, healthcare, transportation, and other community supports.
  • Provide services in community settings and participants' home environments when clinically appropriate and consistent with safety requirements.
  • Support participants in developing and maintaining skills that promote independence, stability, and successful community living.
  • Recognize and respond appropriately to behavioral health concerns, including utilizing effective de-escalation and crisis-intervention skills.
  • Complete timely and accurate documentation in electronic health records and other required systems.
  • Maintain confidentiality and comply with applicable organizational, state, and program requirements.
  • Participate in team meetings, care coordination, care planning, and other program activities.
  • Conduct outreach and community engagement, including participation in community-based events and activities designed to increase awareness of services and support program growth.
  • Develop relationships with community organizations, referral sources, and other partners that can support participant outcomes and program development.
  • Perform other duties as assigned.
Required Qualifications & Skills
  • Demonstrated experience providing case management, direct support, or behavioral health services to individuals with severe and chronic mental illness.
  • Experience working with individuals enrolled in or receiving services similar to PRP and/or MTS is strongly preferred.
  • Strong interpersonal, verbal, and written communication skills.
  • Excellent engagement and rapport-building abilities.
  • Ability to work effectively with individuals who may have complex behavioral health and social service needs.
  • Demonstrated ability to remain calm, flexible, and professional in challenging situations.
  • Strong de-escalation and conflict-management skills.
  • Comfort providing services in community-based settings and participants' homes, subject to appropriate safety practices and protocols.
  • Proficiency with technology, including basic Microsoft Office or comparable office productivity software.
  • Experience using electronic health records (EHRs) and completing electronic documentation.
  • Ability to work independently while also functioning effectively as part of a multidisciplinary team.
  • Strong organizational skills and the ability to manage multiple participants, priorities, and deadlines.
  • Valid driver's license and a clean driving record.
  • Reliable transportation and willingness/ability to travel throughout the service area as required by the position.
Preferred Qualifications
  • Prior experience in PRP, MTS, behavioral health rehabilitation, community mental health, or related services.
  • Peer Support Specialist certification or other relevant behavioral health credential.
  • Familiarity with Maryland's behavioral health system and available community resources.
  • Experience assisting individuals with public benefits, housing, healthcare, employment, transportation, and other social service needs.
  • Experience conducting community outreach, networking, and engagement activities.
  • Experience working with individuals with complex or co-occurring needs.
