Job summary

IMind Health is seeking a Community Support Worker to provide direct support and case management in Lanham, Maryland. The role involves working in community settings and, when appropriate, in participants’ homes, while maintaining professional boundaries and safety protocols.

Candidates should have strong engagement and documentation skills, with experience in behavioral health services for individuals with severe mental illness. A valid driver's license and reliable transportation are required.

Qualifications

• Experience providing case management or behavioral health services to individuals with chronic mental illness.
• Experience with PRP/MTS or similar programs preferred.
• Strong interpersonal, verbal, and written communication skills.
• Ability to work in community-based settings and in participants' homes when safe.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct support, case management, and rehabilitation services to program participants.
• Develop and maintain professional relationships with participants and build trust.
• Assist participants with needs assessment, goal setting, and connecting to resources.
• Transport participants to appointments and community activities as needed.
• Complete timely documentation in electronic health records and other systems.
• Maintain confidentiality and comply with program requirements.
• Engage in team meetings and care coordination activities.
• Conduct outreach to community organizations to support participant outcomes.
