Job summary

IMind Health is seeking a Community Support Worker in Lanham, Maryland to provide direct support, case management, and rehabilitation services to program participants enrolled in PRP and/or MTS. This contract role emphasizes community-based work, including occasional home visits, while maintaining professional boundaries and safety protocols.

The ideal candidate will have experience working with individuals with severe mental illness, strong engagement skills, and the ability to coordinate care

Qualifications

• Experience providing case management or behavioral health services to individuals with severe and chronic mental illness.
• Experience with PRP and/or MTS-like services is strongly preferred.
• Strong interpersonal, verbal and written communication, and engagement abilities.
• Ability to work in community settings and home environments with safety practices.

Responsibilities

• Provide direct support, case management, and rehabilitation services to participants.
• Build professional relationships with participants and maintain trust.
• Identify needs, set goals, and connect to appropriate resources and services.
• Transport participants to appointments and community activities as appropriate.
• Document timely and accurate information in EHRs and other systems.
• Participate in team meetings, care planning, and outreach activities.
