Job summary

Collaborative-Support-Programs (CSPNJ) seeks a full-time Support Worker to provide direct support, skill development, and community linkages to residents with psychiatric and/or substance use challenges. The role focuses 70–80% on field visits, aligning services with individual plans and regulatory standards.

Strong communication and trauma-informed care are essential, with opportunities for on-call rotation compensation.

Qualifications

• Clear, concise, timely, and accurate progress notes.
• Ability to support daily living activities and develop skills.
• Strong communication and collaboration with team and residents.

Responsibilities

• Provide direct support and skill development to residents.
• Meet with residents on service plans and assess needs (70–80% field work).
• Complete progress notes in compliance with regulatory standards.
