Job summary

Collaborative-Support-Programs in New Jersey seeks a full-time Support Worker to provide direct support, skill development, and resource linkages for residents with psychiatric and/or substance use challenges. You will largely work in the field, meeting service plan goals and assessing needs to promote independent living.

Progress notes must be completed promptly and per regulatory standards. You will coordinate with community providers, advocate for clients, and participate in rotating on-call

Qualifications

• Direct support and skill development for residents with mental health or substance use challenges.
• Ability to prepare clear progress notes meeting regulatory standards.
• Experience engaging with community resources and advocates.

Responsibilities

• Provide direct support and develop residents' skills to live independently.
• Field-based work 70–80%, completing service plans with residents.
• Document progress notes timely and accurately per regulations.
• Build community partnerships to expand access to housing, healthcare, and services.
