Job summary

The Centers in Cleveland, OH is seeking a full-time Professional I, Behavioral Health Specialist/Case Manager to provide individualized behavioral health support and case management to clients in recovery.

The ideal candidate has a relevant degree and a valid Ohio driver's license with access to an insured vehicle; you will work closely with a multidisciplinary team to assess needs and coordinate care from intake to discharge.

Qualifications

• Bachelor's, Associate's, or high school diploma with 3 years in social services.
• Valid driver's license in Ohio with access to insured vehicle.
• Ability to work well with a team and engage clients in assessment and goal-setting.

Responsibilities

• Provide individualized behavioral health support and case management.
• Coordinate care plans from initial contact through end of service.
• Engage clients in mutual assessment of problems and goal-setting.
