Job summary

AccessPoint RI in Rhode Island seeks a Licensed Practical Nurse to conduct annual community-based health assessments and provide medical management for Personal Choice Program participants.

You will liaison with the participant's Primary Care Physician, document assessments accurately, review long-term care needs, ensure licensing guidelines are met, and support program staff with budgets, ISSP approvals, and incident reporting per protocol.

Qualifications

• Active Rhode Island LPN license.
• 2–3 years in Home and Community Based or rehabilitation nursing.
• Data entry and computer skills.
• CPR certification.

Responsibilities

• Conduct annual community-based health assessments.
• Provide medical management and education services as needed.
• Liaise with the participant's Primary Care Physician for medical concerns.
• Report health and safety concerns to appropriate authorities per policy.
• Maintain written documentation of assessments (functional, mobility, health).
• Review long-term care needs and assist in gathering documents for licensing bodies.
• Ensure records meet licensing guidelines and provide budgets/ISSP approvals.
• Collaborate with program staff to coordinate incidents and follow confidentiality rules.
