Job summary

AccessPoint RI is seeking a compassionate Licensed Practical Nurse (LPN) to join our residential nursing team. You will provide clinical nursing care across multiple homes, educate DSPs on medication administration, and collaborate with interdisciplinary staff to maintain health and safety for adults with developmental disabilities.

In this hands-on role, you’ll monitor health changes, support individualized care plans, and ensure consistent adherence to medical protocols.

Qualifications

Must obtain an LPN Certification or Diploma from an accredited school.
Two to four years of LPN experience preferred.
Valid driver’s license and ability to meet agency transportation requirements.
Ability to lift up to 50 lbs and up to 90 lbs with assistance.
Experience with adults who have developmental disabilities.

Responsibilities

Provide competent professional medical services to individuals in day and residential services.
Deliver nursing care using the nursing process (assessment, planning, intervention, implementation, evaluation).
Coordinate services with Physical/Occupational/Speech therapy as needed and with DSPs.
Educate and support DSPs by providing medication administration training and safe health practices.
Notify clients' families of incidents and liaise with healthcare and agency teams as required.
