Job summary

Rehabilitation Hospital of Rhode Island is seeking a Licensed Practical Nurse (LPN) to provide direct patient care under RN supervision, delivering treatments and supporting interdisciplinary care planning. The role emphasizes assessment, medication administration, documentation, and care planning to improve patient outcomes.

Qualifications

• Education: Graduation from an accredited BSN, ADN, or Nursing Diploma program.
• Experience: Minimum six months of Medical/Surgical experience in an acute care setting preferred.
• License: Active Rhode Island LPN license (single-state or compact) required; relocation may necessitate PSOR update before employment begins.
• Certifications: CPR/BCLS certification required; IV Certification highly preferred or must be obtained within 6 months.

Responsibilities

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides care appropriate to condition and age of the patient.
• Performs timely and accurate documentation relating to medical necessity in the medical record.
• Administer medications safely, accurately, and based on patient age and clinical condition.
• Documentation relating to medical necessity in the medical record.
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
• Performs timely and accurate QI assessments.
• Assist the RN with initial assessments and patient reassessments per policy.
• Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
• Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
