Job summary

HireCare Services, partnering with a leading skilled nursing facility in Warwick, RI, is seeking a compassionate Licensed Practical Nurse (LPN). You will deliver direct nursing care, support RNs, administer medications, and help develop individualized care plans to enhance resident well-being.

Must have strong clinical judgment and good communication.
Demonstrated teamwork with nursing staff.
Ability to manage multiple priorities professionally.

Responsibilities

Provide direct nursing care according to policies and care plans.
Assist RNs and other staff with daily duties and resident care.
Administer medications and treatments as ordered.
Monitor residents for changes and report concerns.
Document care accurately and timely.
Collaborate on individualized care plans.
Participate in care conferences and updates.
Promote comfort, dignity, privacy and independence.
Communicate with residents, families and healthcare team.
Follow regulations, safety procedures and policies.
