Job summary

The Jewish Board is seeking a registered nurse to join the ACT team, delivering a range of treatment for clients with SPMI/SUD in community settings. The RN conducts initial and ongoing nursing assessments, coordinates medical care, and administers medications in collaboration with psychiatrists and providers.

With primary duties in the community (about 80%), the role supports recovery through health education, referrals, and care coordination, while maintaining charting and regulatory

Qualifications

• RN or BSN in nursing and New York State registration.
• Experience in a residential setting or mental/behavioral health field strongly preferred.
• Experience with electronic health records and standard MS Office tools (Word, Excel, Outlook).

Responsibilities

• Carry a caseload and perform initial and ongoing nursing assessments.
• Conduct weekly visits for clients in shelters or community settings; develop treatment plans with the team.
• Administer prescribed treatments and medications; coordinate with psychiatrists, PNP, and other providers.
• Coordinate and deliver medications to clients in residences; manage medication counts and referrals.
• Provide health education, harm-reduction counseling, and staff training; facilitate groups.
• Charting and documentation; monitor lab results and communicate changes to the care team.
• Act as medical representative at interdisciplinary mental health team meetings.
