Job summary

The Betty Hardwick Center seeks a dedicated nurse to serve as the primary RN for the ACT Team, coordinating and implementing the physician's plan of care at the weekly medication clinic. This role requires collaboration with Center staff, outside agencies, and civil authorities to support ACT clients and others in Level of Care 3.

Responsibilities include maintaining a current community resource network, providing telephone consultations to Case Managers, and delivering services across office,

Qualifications

• Experience in mental health/intellectual and developmental disabilities preferred.
• Must have a valid Texas Driver's License and be an insurable driver. Driving is an essential function and employees must maintain an acceptable driving record/insurability while employed.

Responsibilities

• Serve as the primary nurse for the ACT Team and coordinate/oversight/implementation of the physician's plan of care for the weekly medication clinic.
• Coordinate services with Center staff, outside agencies, and civil authorities to support ACT Team clients and individuals in Level of Care 3.
• Assist in maintaining an up-to-date community resource network.
• Be available via phone for consultation with Case Managers on medical/nursing issues for ACT patients.
• Deliver services in the office, in the community, and at other Center sites within the catchment area; ensure documentation meets state and federal standards, including Medicaid requirements.
• Strive to meet monthly performance targets with professional interaction with consumers, family, staff, and outside agencies.
