Job summary

Jewish Board of Family and Children's Services seeks an RN to join our ACT team across New York City. The RN provides treatment for clients with SPMI/SUD, conducts initial nursing assessments, and performs ongoing health monitoring in the community, coordinating with psychiatrists and medical providers.

The role is primarily community-based (about 80% field, 20% office). Responsibilities include medication administration, medical case management, health education, and collaboration with shelters

Qualifications

• RN or BSN and New York State registration.
• Experience in residential or behavioral health preferred.

Responsibilities

• Carry a caseload and perform initial and ongoing nursing assessments.
• Conduct weekly visits for clients in shelters or in the community.
• Develop treatment plans and address medical diagnoses with education.
• Administer prescribed treatments and support self-administration.
• Coordinate and deliver medications to client residences or shelters.
• Make referrals, schedule appointments, and accompany clients when required.
• Act as medical representative in interdisciplinary meetings.
• Document care and monitor lab results with clinicians.
• Promote health education and train staff on health practices.
