OneWell Health Care is seeking a Direct Support Professional in Perrysburg, OH to provide person-centered support to individuals with IDD and Autism in home and community settings. You will assist with daily living activities, promote independence, and ensure health and safety.

You will document ISPs, help with personal care, monitor well-being, and communicate with families and care teams. Training on DODD, Medicaid HCBS, HIPAA, and agency policies will be provided.

Qualificações

• CPR Certification.
• First Aid Certification.
• Ohio DODD-required training.
• Medication administration certification when assigned.
• Additional agency-required training.

Responsabilidades

• Assist with daily living activities and personal care, promoting independence.
• Provide medication support when assigned.
• Support skill development and community participation.
• Implement ISPs and behavior support strategies.
• Monitor health and safety needs and maintain documentation.
• Communicate with families and care teams; participate in training and quality improvement.
• Ensure services are delivered with dignity and respect, in compliance with policies.
