Job summary

Chrysalis Center, Inc. seeks a dedicated Community Support Specialist to provide case management and direct care in community settings. You will guide individuals with complex psychiatric, substance use, medical, social, and legal challenges to improve daily living skills and support recovery.

Bachelor’s degree plus 1–2 years in mental health; bilingual Spanish preferred. Proficiency with Microsoft Office and EHR; crisis de-escalation and team collaboration skills; valid driver’s license may be

Qualifications

Bachelor’s degree in mental health or related field is required.
Experience in mental health and substance use services preferred.
Bilingual Spanish is preferred to support a diverse client base.

Responsibilities

Provide a full range of community support services including case management and housing.
Complete rehabilitation assessments, reassessments and update recovery plans.
Engage clients to improve learning, motivation and daily living activities.
Maintain documentation and required paperwork for funder/program compliance.
Provide transportation to clients using agency vehicle when appropriate.
Facilitate self-help and advocacy groups and participate in required meetings.
