Job summary

Quality Care Residential Homes, Inc. is seeking a dedicated Case Manager Coordinator to oversee individualized care plans for adolescents, coordinating services across behavioral health, social services, and community resources to support youth toward independence.

The role requires a degree in a related field, case management experience, a valid driver’s license, and in-person work Monday to Friday. You will collaborate with families, schools, and community partners, and contribute to progress

Qualifications

• Must have a degree in social work, psychology, human services, or related field.
• Experience in a case management or similar role; youth work preferred.
• Valid driver’s license and in-person work Monday–Friday.

Responsibilities

• Participate in case coordination and service plan conferences for youth.
• Provide independent living training.
• Coordinate meetings with partners related to youth care and progress.
• Intake of new youth and maintain monthly progress reports in the information system.
• Ensure deadlines for youth appointments are met.
• Liaise with families, schools, and community partners.
• Assist with employment, programs, and aftercare services.

Job description

We are seeking a dedicated and energetic Case Manager Coordinator to join our team. In this vital role, you will coordinate and oversee individualized care plans for adolescents, ensuring they receive comprehensive support across behavioral health, social services, and community resources. Your leadership will empower young clients to develop essential life skills and successfully navigate their paths toward independence and stability.

Requirements

Must have experience in Case Management role or position. Degree is required in related fields (e.g., social work, psychology, human services, case management, etc.). Prefer to have at least one year of full-time equivalent experience in case management or social work. Work with youth is a plus. Must have a valid driver's license. Must be able to work M-F in-person

Responsibilities

• Participate in the case coordination and service plan conferences for youth.
• Provide independent living training,
• Attend and coordinate meetings with partners related care and progress for youth,
• Intake of new youth
• Monthly Progress reports in Information System
• Critical incident reporting and report writing and entry into State Information Systems
• Quality assurance of youth records
• Meet with youth monthly for progress review
• Ensure deadlines are met for appointments for youth
• Assist with resource finding for youth,
• Liaison with families, schools and community partners
• Assist with finding employment, summer programs, vocational programs
• Assist with aftercare services pre and post discharge
• Provide input to Service Plan/Behavior Plan and 90-day reviews,
• And any other duties as deemed necessary by administration.
