Hope Network in Mount Clemens, MI is seeking a Case Manager to deliver community-based services and home visits, coordinating care to maximize recovery and independence for individuals with mental illness.

You will perform psychosocial assessments, support individualized service plans, and collaborate with hospital and community partners to ensure seamless transitions and compliance with program standards. This role requires empathy, reliability, and strong communication.

Qualifikationen

• Bachelor’s degree in a human services field from an accredited institution.
• Willingness to work in the community and perform home visits.
• Valid Michigan Driver’s License.
• Preferred licensure: LLMSW, LMSW, LMFT or equivalent.

Aufgaben

• Work in the community performing home visits.
• Coordinate services to maximize consumer recovery and independence.
• Provide clinical and program consultation to staff.
• Support individualized planning and PCP participation.
• Perform comprehensive psychosocial assessments.
• Facilitate hospitalizations and coordinate with hospital social workers.
• Coordinate intake, discharge, and transfers.
